(hereinafter referred to as "our company") will adhere to "JIS Q 15001 and Laws and Regulations related to Personal Information Protection" to formulate the "Privacy Policy" to ensure that our company remains a trusted enterprise.
Furthermore, all officers and employees of our company commit to "understanding" and "thoroughly implementing" this policy and declare the establishment, implementation, maintenance, and "continuous improvement" of a management system for personal information protection to achieve the realization of personal information protection.

Furthermore, our company strives to prevent and rectify unauthorized access, loss, destruction, alteration, leakage, loss, and damage to personal information through the implementation of reasonable technical measures. Our company also ensures appropriate supervision of subcontractors when entrusting personal information.

6. Procedures for Disclosure Requests

Our company will appropriately handle procedures related to disclosure requests as required by law. If you wish to request the disclosure of your own information, we will respond within a reasonable timeframe and scope after confirming your identity.

Procedures for Responding to Requests for Disclosure, etc., of Personal Information Subject to Disclosure

If we receive a request for disclosure, etc. (notification of purpose of use, disclosure, correction, addition, or deletion of content, suspension of use, elimination, and suspension of provision to third parties) of personal information subject to disclosure in our possession, we will disclose such information to a reasonable extent (the same applies to disclosure of records provided to third parties). To request disclosure, etc., of personal information, you must submit the following documents (1) and (2) and pay a fee (3).

(1) Our designated request form
Please contact the following inquiries, obtain the "Request for Disclosure, etc., Submission Form," and complete the necessary information.

(2) A document that can confirm your identity
Choose one of the following:

  • A copy of your driver's license
  • A copy of your passport

Or two of the following:

  • Original copy of your seal certificate
  • Original copy of your resident certificate
  • A copy of your health insurance beneficiary's card
  • A copy of your pension book
  • A copy of a public utility (electricity, gas, water) bill or receipt

The identification documents submitted will be used to verify your name and address. Please be aware that this information will be used to manage your requests and their status. You may redact any other information present on the identification documents.

If a representative requests the disclosure of personal information on behalf of the individual, please provide documents confirming the identity of the individual and a letter of proxy.

(3) Fee of 1,000 yen (including tax) (Please pay with a fixed-amount postal money order)
However, exceptions apply in the following cases:

  • ■ Correction, addition, or deletion of personal information
  • ■ Suspension of use, erasure, or suspension of third-party provision of personal information
  • Please make the payment using a fixed-amount postal money order with a face value of 1,000 yen, which is available at post offices. Note that there is a 100 yen issuance fee for the 1,000 yen fixed-amount postal money order.
  • Leave the "Designated Recipient's Address and Name" section of the fixed-amount postal money order blank.

(4) Submission Method
Once you have gathered the necessary documents, please send them to the contact point for inquiries regarding our company's personal information protection policy via mail. Please note that we cannot accept requests made in person at our office.
